The Accountable Care Learning Collaborative (ACLC) serves as the foundation for health care stakeholders across the industry to collaborate on improving the care delivery system. With a focus on changing the way providers deliver care, members share their personal experiences regarding the solutions and competencies needed under all forms of accountable care payments within the ACLC’s seven core workgroups. They also receive information crucial to running their organization in a new value-based economy.

CAPC has partnered with ACLC to disseminate information to ACLC members about the critical role that palliative care services play in improving value for the population of patients living with serious illness. Through published case studies and presentations, ACLC members learn about the value case and the evidence base for palliative care, and receive guidance on implementing palliative care services.
