Health systems, health insurance plans, accountable care organizations (ACOs), and other risk-bearing entities that are working to improve the quality of care while managing its cost need to pay special attention to individuals living with serious illness. CAPC resources help these organizations to proactively identify this sub-population; improve the impact of case management; and build benefits and programs to match services to needs.
