PACE – Program of All-inclusive Care for the Elderly – is a comprehensive payment and care model for Medicare and Medicaid beneficiaries with functional limitations, using an inter-professional team to provide holistic, high-quality care. The National PACE Association provides education, advocacy, data, technical assistance, and networking to PACEs throughout the country.

CAPC collaborated with the National PACE Association (NPA) to create the Palliative Care Playbook for PACE, a step-by-step guide to ensure screening for distress, meaningful conversations with participants and family members, and skilled pain and symptom management. CAPC and NPA have also provided joint education on reducing disparities in the care of people with serious illness, and continue to provide joint education to PACEs and palliative care champions.
