50 y/o father of three children, with colon cancer needing a referral to interdisciplinary palliative issues.  He presented with significant pain complaints, self-reported stress with family dynamics, and limited financial resources.  The patient lives 3 hours away and must coordinate palliative care appointments with treatment appointments.  Rapid programmatic growth has negatively impacted access for initial appointments in the Palliative Care Clinic, resulting in a 3-4 week appointment availability which caused extreme distress for the patient and referring provider.  Presenters will review how leaders engaged the interdisciplinary team to redesign the care delivery model by utilizing data and LEAN principles to identify stress points and find creative solutions aimed to increase appointment access, decrease no show rates, and optimize clinic workflow.
