What You’ll Learn

Expanding palliative care to new patient populations or care settings requires a strategic approach. Whether you are in a community-based organization working to improve access to palliative care in the areas you serve, or in a hospital or health system aiming to enhance continuity of care from hospital to home, careful planning is essential. Given complexity and costs, it is critical to explore potential partnerships across teams and community organizations. This course provides a framework for leaders to think across the continuum of care, offering strategies for building or collaborating to expand services.

Learning outcomes for this course include:

  1. Summarize a process for evaluating whether to build new services or partner to expand services
  2. Identify 3 foundational metrics that are essential to strategic planning that consider realistic assumptions regarding cost, capacity for new patients, and results
