Improving Post-Heart Attack Care for Hispanic Patients: A Culturally Tailored Discharge Model

Overview
Hispanic patients experience disparities following acute myocardial infarction (AMI), including higher readmission rates and lower referral to cardiac rehabilitation. Language and communication barriers and challenges navigating follow-up care can make the transition from hospital to home particularly difficult.
The Hispanic Acute Myocardial Infarction Discharge Intervention Study (HAMIDI) is a culturally tailored, six-month program that connects Hispanic patients with Spanish-speaking cardiologists, virtual group education on nutrition and physical activity, and hands-on care coordination. By integrating language-concordant care, culturally responsive education, and navigation support, HAMIDI aims to improve continuity of care and recovery following AMI.
