Improving Access to Pulmonary Rehabilitation for Hispanic and Black Patients with COPD Through Telehealth

Overview
The Telehealth-delivered Pulmonary Rehabilitation (TelePR) program was created to improve access to pulmonary rehabilitation (PR) for underserved Hispanic and Black patients with chronic obstructive pulmonary disease (COPD), particularly those recently hospitalized for COPD exacerbations. Despite its effectiveness, PR remains substantially underutilized due to barriers including limited access, transportation challenges, financial strain, and debilitating respiratory symptoms.
TelePR offered 16 virtual sessions over 8 weeks focused on exercise, symptom management, and social support. Patients received home exercise equipment, vital sign monitors, and a WiFi-enabled tablet to support participation. A bilingual respiratory therapist led sessions, while a social worker helped address insurance, transportation, scheduling, and other social needs. The program was primarily home-based, with community centers available for patients who faced space, safety, or internet barriers.
