Responsible for management of palliative care services for Santa Clara Family Health Plan (SCFHP) members. As a SCFHP member advocate the Palliative Care Liaison facilitates communication and coordination among all participants of the health care team. Primary duties are to ensures that needed services are provided that promote high-quality care for members facing serious illness.

Essential Duties and Responsibilities
1. Facilitate referrals from plan to HBPC providers
2. Oversight of palliative care providers
3. Maintain and update as needed policies and procedures related to palliative care benefit
4. Act as a bridge between inpatient palliative services and home-based services, including telephonic and face-to-face initial PC consultation
5. Act as primary coordinator of services for our members who are nearing end-of-life

Requirements – Required (R) Desired (D)
1. Bachelor’s Degree in Nursing with CHPN certification or equivalent graduate-level training, or certification (R)
2. Minimum of three to five years of professional work experience to include at least one-year of palliative care or hospice nursing experience or equivalent. (R)
3. Current California Nursing license, in good standing (R)
4. General knowledge of managed care principles and practices (D)
5. Strong clinical knowledge and critical thinking skills with the ability to assess individualized whole-person care needs necessary to develop and effective plan of care (R)
6. Ability to work within an interdisciplinary team structure and also function independently within a fast-paced environment while managing multiple priorities and meeting deadlines (R)
7. Adept at using computer systems both Microsoft Office applications and SCFHP specific case management programs (R)
8. Excellent communications skills both oral and written, as is mandated by nursing scope of practice (R)
9. Valid and current Driver’s license (R)
