The Opportunity

SSM Health Dean Medical Group is actively recruiting a full-time (1.0 FTE) Palliative Care Nurse Practitioner to join our inpatient palliative care program in Madison, WI.
The Palliative Care Nurse Practitioner works in collaboration with the Palliative Care Consultation Service Medical Director and other interdisciplinary team members and is accountable for providing specialty palliative care to patients and their families in both consultative and co-management roles without direct supervision. He or she also assists with staff and community education, and participates in hospital and health system initiatives pertinent to the field, including work on primary palliative care, program development, and hospital committees.

Minimum Qualifications:
• Advanced Certified Hospice and Palliative Nurse (ACHPN), if not already obtained, within two years of hire
• Two years of clinical experience in hospice, geriatrics, oncology and/or primary care
The ideal candidate will be patient-centered, service-oriented, and hardworking and will demonstrate the competencies of a palliative care provider.

SSM Health St. Mary’s Hospital - Madison has been serving South Central Wisconsin since 1912. The hospital is a tertiary care facility with 350 beds that is used by SSM Health Dean Medical Group physicians. The hospital and the medical group work in a complementary manner as part of SSM Health Wisconsin to provide excellence in patient care.
