JOB SUMMARY
The Nurse Practitioner Supervisor provides both direct patient care and leadership oversight for the NP members of the Maimonides Health palliative medicine division--spanning Maimonides Medical Center, Maimonides Midwood Community Hospital, and Maimonides Cancer Center. In addition to carrying an independent clinical caseload, the Supervisor is responsible for the day-to-day operational management of the division's nurse practitioner staff, including scheduling, performance monitoring, mentorship, and staff development; for coordinating consult triage and workflow across all sites; and for leading quality improvement initiatives that advance the standard of palliative care delivered throughout the health system. This role requires advanced clinical judgment, strong leadership and communication skills, and the ability to represent the division in interdisciplinary, administrative, and institutional settings.
REQUIRED LEADERSHIP COMPETENCIES
1. Communication: Demonstrates strong written and verbal skills in reports, correspondence, and presentations. Ensures staff are kept informed of Medical Center and departmental priorities, and communicates the division's goals and objectives.
2. Professional Development: Participates in conferences, webinars, and professional development activities (e.g., CAPC, AAHPM, VitalTalk) to maintain licensure and remain current in palliative care practice.
3. Staff Development: Mentors and coaches NP staff on clinical practice and communication skills; ensures staff receive mandated in-servicing in safety, quality assurance, risk management, customer service, and infection control.
4. Problem Solving: Exercises strong analytical skills in staffing, scheduling, and clinical decisions, with attention to quality, cost containment, and impact on other departments.
5. Customer Service Management: Incorporates the Medical Center's customer service and organizational goals into divisional policies and workflows; serves as a role model to staff and peers.
6. Project Management: Leads and participates in divisional quality improvement initiatives and multidisciplinary committees, bringing together different views and skills toward timely completion of objectives.
7. Clinical Leadership: Provides clinical supervision to the division's NP staff, serves as a clinical resource for complex cases, and may provide leadership for hospital-wide, palliative-related committees.
SUPERVISORY & LEADERSHIP RESPONSIBILITIES
• In collaboration with the Division Chief, provides oversight and day-to-day supervision of the division's nurse practitioners (currently 4 NPs) across Maimonides Medical Center, Maimonides Midwood Community Hospital, and Maimonides Cancer Center.
• In collaboration with the Division Chief, monitors individual and team performance and conducts and documents annual performance evaluations, identifying goals and areas for professional growth.
• Mentors and coaches NP staff on clinical practice, communication skills (e.g., goals-of-care conversations), and professional development; serves as a clinical resource for complex cases.
• Participates, in collaboration with the Division Chief, in interviewing, hiring, and onboarding of new NP staff for the division.
• Prepares and maintains staff schedules, including vacation, holiday, and weekend coverage; ensures adequate NP coverage across all sites; ensures staff time is entered and approved in Kronos on a weekly basis.
• Establishes, maintains, and updates departmental policies, procedures, and quality assurance standards for the NP members of the division, in collaboration with the Division Chief.
• Carries a clinical caseload that is proportionally reduced relative to staff NPs, in recognition of the time required for supervisory, administrative, and quality improvement responsibilities.
• Leads and participates in divisional quality improvement (QI) initiatives, tracking outcomes (e.g., consult volume, response times, documentation compliance) and implementing practice improvements.
• Prepares cases for peer review and supports the division's participation in institutional quality and compliance programs.
• Serves as a liaison between the NP division and hospital administration, physician leadership, and interdisciplinary teams; communicates and educates hospital staff on the division's goals and objectives.
• Represents the division at relevant meetings, webinars, and conferences and disseminates relevant updates to staff.
• Maintains compliance with organizational credentialing, licensure, and regulatory requirements for the division's NP staff.
CLINICAL & OPERATIONAL RESPONSIBILITIES
• Triages incoming palliative care consult requests (via pager or other means) and assigns initial and follow-up consults to team members across all covered sites.
• Ensures the division's daily sign-out, consult log, and patient tracking systems (e.g., SharePoint) are accurately maintained, and monitors related dashboards/metrics.
• Sees initial and follow-up palliative care consults as needed: performs comprehensive health assessments and histories; participates in advance care planning; assesses and manages symptoms; and coordinates plans of care with the treatment team.
• Facilitates family meetings and goal-of-care discussions with patients, families, and the interdisciplinary team (medicine, nursing, social work, chaplaincy).
• Obtains and documents advance directives, health care proxies, surrogate designations, and eMOLST forms; ensures appropriate scanning/filing in the medical record system as needed.
• Collaborates with case management and social work on disposition planning; initiates referrals to inpatient hospice at Maimonides as needed.
• Documents all clinical findings, discussions, and plans of care in the electronic medical record in a timely manner.
• Serves as a resource to nursing staff and other clinical team members on palliative-specific topics, including hospice referral, PCA use, high-dose opioid management, and palliative extubation.
PROFESSIONAL DEVELOPMENT
• Maintains active membership and engagement with relevant professional organizations (e.g., CAPC, HPNA, AAHPM) and completes related educational modules and conferences.
• Maintains and pursues relevant specialty training.
• Maintains all required licenses, certifications, and continuing education (CME/CEU, BCLS/ACLS as applicable), and keeps certificates of completion current with institutional compliance requirements.
BASIC COMPETENCIES
Education/Licenses:
• Master's degree in nursing with a focus on primary health care and completion of an approved subspecialty
• Current NYS licensure as an RN, and current NYS registration as a Nurse Practitioner
• Active DEA license to prescribe controlled substances
• Certification as an Advanced Certified Hospice and Palliative Nurse (ACHPN) strongly preferred
• Minimum of 3–5 years of clinical experience in palliative medicine and, ideally, fellowship training
• Minimum of 1 year of supervisory experience required
Skills:
• Demonstrated leadership, mentorship, and conflict-resolution skills
• Advanced clinical assessment and decision-making skills in high-acuity, high-complexity situations
• Proven ability to manage staff scheduling, performance evaluation, and personnel matters
• Excellent oral and written communication skills, particularly in sensitive and emotionally complex conversations
• Comfort with high-dose opioid management, PCA systems, and palliative extubations
• Ability to travel and work across multiple sites (Maimonides Medical Center, Midwood Community Hospital, and Cancer Center) as needed
• Strong computer skills highly preferred (e.g., electronic medical records, Microsoft Office, scheduling and timekeeping systems, SharePoint, WebEx)
• Fluency in English required; proficiency in an additional language preferred
JOB SETTING / PHYSICAL DEMANDS
• Patient care areas across all covered sites, including inpatient units, ICUs, ambulatory clinics, and the cancer center; this may require periods of prolonged standing/walking
• May lift, position, push, and/or transfer patients
• May handle emergency or crisis situations
• Protective clothing may be required: mask or face shield, gloves, impermeable gown, particulate respirator
• Frequent contact with patients, families, staff, and administration in potentially emotionally intense and clinically complex situations
• Full-time schedule with flexibility required to meet staffing, coverage, and patient care needs across sites, including some weekends/holidays
