Physician – Palliative Care | Sunnyvale, CA
Join Palo Alto Foundation Medical Group (PAFMG), one of the nation's premier physician-led, multi-specialty medical groups, as a Board Certified/Board Eligible Palliative Care Physician in Sunnyvale, California.
As part of our nationally recognized Palliative Care and Support Services (PCSS) program, you'll work in a collaborative, multidisciplinary environment dedicated to improving the quality of life for patients and families facing serious illness. Our innovative outpatient program delivers care wherever patients need it most—including the clinic, home, assisted living communities, skilled nursing facilities, and through telemedicine.
Opportunity Highlights
- Join an established, nationally recognized outpatient palliative care program
- Collaborate with experienced primary care physicians, specialists, nurses, social workers, chaplains, and other interdisciplinary team members
- Develop comprehensive care plans, provide patient and family education, and deliver ongoing symptom management and supportive care
- Resource-rich practice environment supported by the Palo Alto Medical Foundation and Sutter Health
- Full-time schedule consisting of 35 patient contact hours per week (5-day workweek)
- Excellent work-life balance with no night shifts or weekend call
Qualifications
- Fellowship training in Hospice and Palliative Care Medicine
- Board Certified or Board Eligible in Hospice and Palliative Care Medicine
- Eligible for California medical licensure
