This APP plays a vital role in supporting physicians and delivering high-quality patient care within the outpatient setting. This full-time position is primarily office-based and combines direct patient care with clinical coordination responsibilities to ensure timely and comprehensive care for geriatric and palliative care patients.

Essential Duties and Responsibilities
• Manage and respond promptly to electronic in basket messages, patient communications, test results, and other clinical tasks for Geriatrics and Palliative Care providers.
• Evaluate and treat established patients on an urgent basis when new symptoms or clinical concerns require immediate assessment and intervention.
• Provide comprehensive follow-up care for patients with complex geriatric and palliative care needs.
• Collaborate closely with physicians, nurses, care coordinators, and other members of the interdisciplinary care team to ensure continuity of care.
• Document patient encounters and clinical activities accurately and in a timely manner within the electronic medical record.
• Participate in care planning, patient education, and coordination of services to support optimal patient outcomes.
Work Environment
• Full-time position with the majority of work performed within the medical office building.
• Approximately 50% of work time involves direct, face-to-face patient care.
• Remaining responsibilities include clinical coordination, patient communication, documentation, and in basket management activities.
