Description
Position Purpose
The inpatient Palliative Care physician will primarily provide consultative care for patients with advanced, serious illness in the acute care setting including the medical center and the Transition Care Unit (TCU). In addition, this role will actively support the hospice program including the general inpatient level of care (GIP), participate in after-hours provider on-call coverage, and support the community-based palliative care services, as needed. This physician will also work closely with an interdisciplinary team and assist in coordinating care during admission and when transitioning care across the continuum.
Position Details:
General duties:
Provides consultative care to patients with advanced, serious illness (such as advanced cancer, ESRD, CHF, COPD, dementia, ESLD…) through symptom and pain management and must be comfortable with facilitating difficult medical decisions with patients and family surrounding goals of care, advance care planning and end of life care discussions.
Facilitates discussion on Advance Directives and POLST.
Facilitates daily hospital palliative care rounds.
Has knowledge and ability to discuss the End-of-Life Option Act.
Collaborates closely with physicians and other specialists in coordinating concurrent treatments and recommendations reflected in the plan of care.
Works with a team from different disciplines to provide transdisciplinary care.
Works with different EMR systems, primarily with Cerner.
Documents patient encounters, interventions and care plan in an accurate, clear and timely manner.
Attends and participates in the Palliative Care interdisciplinary care team (ICT/IDT) meetings, as invited, and Palliative Care Practice Operating Committee (POC).
Assists with developing best practices for patient-physician communication and interdisciplinary teamwork, including ways to design and implement the best possible patient experience and outcomes.
Works closely with Palliative Care and Hospice leadership teams
Coordinates care and effective communication with other departments and agencies such as Home Health, Case Management, Social Work, Hospice, Radiation Oncology, primary care providers, specialists and etc.
Sees an average of 5-7 patients a day.
Works predetermined hours and provides timely notification to the team for schedule changes.
Attends and acts as Palliative Care representative in specialty committees and departmental interdisciplinary rounds (ED, ICU, Oncology, etc), as requested.
Establish Quality Assurance process oversight for inpatient palliative care and reports on this data quarterly.
Participates in staff in-service, community events and other educational opportunities to advance Palliative Care philosophy and services.
Demonstrates a high degree of personal integrity and loyalty to the Torrance Memorial Healthcare System.
Other Palliative Care & Hospice duties:
Provides cross coverage support for the outpatient Palliative Care and Hospice teams, as needed.
Supports hospital GIP service line.
Participates in after-hours provider on-call duties for both Palliative Care and Hospice Care
Transitions care to hospice when appropriate and can continue as Attending Physician for patients placed under Hospice Care.
Requirements:
Active CA state license
BCLS/ALS certified
Preferences:
BC Internal Medicine, Hospitalist
BC/BE Hospice & Palliative Care
Salary Range: $250,000 - $300,000 / Full-Time Compensation
Policy: We offer a competitive total compensation and benefits package to our physicians. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Total compensation takes into account the wide range of factors that are considered in making compensation decisions including knowledge/skills; relevant experience and training; education/certifications/licensure; and other business factors. This total pay range includes incentive payments that may be applicable to this role.
