Monument Health is seeking a dedicated and compassionate LPN to join our Ambulatory Palliative Care team in Rapid City, SD. This is an opportunity to be part of an interdisciplinary care team that helps patients and families facing serious illness live fully and comfortably.
About the Role:
As a key member of our outpatient palliative care program, you will work closely with physicians, advanced practice providers, social workers, and chaplains to deliver high-quality, person-centered care. Your work will focus on symptom management, care coordination, and providing support to patients and families navigating complex health conditions.
Key Responsibilities:
• Provide direct clinical support and education to patients and caregivers
• Collaborate with the interdisciplinary team to develop and implement care plans
• Monitor and document patient progress, symptoms, and response to treatments
• Facilitate communication between patients, families, and healthcare providers
Qualifications:
• Current LPN license in South Dakota or a compact state
• BLS certification required
• Prior experience in palliative care, hospice, or a related setting is preferred but not required
• A commitment to compassionate, holistic care
