PURPOSE SUMMARY

Ensures that the psychosocial needs of the hospice patient and family are addressed in compliance with all standards of care to include Medicare Conditions of Participation, State of New Mexico Licensing Division, NHPCO, agency policies and procedures, and standards related to each individual discipline represented on the IDT.

ESSENTIAL DUTIES:
Ensures that patient and family psychosocial needs are assessed and that appropriate interventions are included in the plan of care to address any identified issues.

Assesses and helps strengthen as needed patient's and family's coping skills.

Assesses patient/family needs related to the responsiveness of their environment and connects the patient/family with community resources as required for financials needs, caregiver needs, safety needs, etc.

Ensures all applicable financial information is gathered and submitted to the billing office for placement for those patients being considered for La Posada in-patient hospice.

Ensures discharge planning begins on the day of admission.

Performs prospective/admission visits when requested.

Completes a bereavement assessment during the comprehensive assessment.

Ensures information for final arrangements is documented and assistance in planning for final arrangements is provided.

Serves as a member of the IDT and actively participates in developing/updating the care plan.

Performs the Comprehensive Assessment within 5 days of the patient's admission and completes documentation of the assessment.

Ensures that the Comprehensive Assessments for each patient are completed at least every 15 days.

Completes the plan of care within 5 days of the admission and updates at least every 15 days thereafter and as the patient's assessed condition changes.

Ensures that the plan of care reflects the goals of the patient and family and that patient/family understanding of the plan of care is documented.

Ensures that all plans of care are individualized, reflect the current status of the patient/family, and are a direct link to the needs of the patient/family as identified on the most recent comprehensive assessment.

Ensures documentation paints the picture of the patient's current condition.

Documents all patient-related contacts, including phone calls to all members of the IDT.

Ensures that documentation on every patient visit and interaction is completed on a timely basis through charting and ensures that documentation includes data elements required for measurements of outcomes.

Ensures that the actual frequency and scope of services delivered match those stated in the plan of care and that the patient level of care is appropriate.

Ensures that appropriate interventions as identified on the plan of care are implemented in a timely manner and in compliance with all applicable standards of care.

Maintains strict confidentiality and compliance with HIPAA regulations for all patient/family/caregiver information.

21 . Follows guidelines for standard and contact precautions.

Monitors consumption and appropriate use of supplies and equipment to ensure effective and efficient use of agency resources.

Ensures that the electronic medical record forms and timesheets are completed and locked within 24 hours.

Assists with admissions of new patients as requested.

Participates in clinical on-call rotation for holidays and night/weekend coverage in emergencies as assigned.

Must participate in quality assessment and performance improvement activities and respond promptly to compliance rules, regulations, and guidelines.

Ensures implementation of and compliance with all agency policies and procedures, licensing and certification rules, regulations, and guidelines.

Adheres to organization's Code of Conduct and reports any potential violations of policy, procedure, or regulation.

Participates in staff training, mandatory meetings, and development of educational programs.

Promptly reports facility and equipment concerns via maintenance connection software.

Actively participates in events that are designed to provide education to the community at large and/or events designed to raise funds for the organization as requested.

Act as improvement team member, participating in quality assessment and performance improvement activities as needed.

Identify and report areas for improvement and concerns for safety of patients, family and staff and suggest alternative ways of intervening that may improve outcomes based on knowledge and familiarity of situations.

Recognize that quality improvement is part of everyone's job responsibilities and stay abreast of performance improvement projects and their outcomes.

Ensure all documentation is complete and all visits are locked and closed within 24 hours of patient visit.

Will follow the Mesilla Valley Hospice Social Work Visit Structure on every patient visit.

If visiting facility patients, you will follow each facility protocols related to documentation requirements and plan of care meetings.

Coordinates with MSW to ensure that supervision as required by regulatory guidelines occurs on an ongoing basis if needed.

Will cover for other social workers in the field/hospice in-patient unit (IPU) as needed.

Ill. ADDITIONAL DUTIES. Other Mesilla Valley Hospice and Palliative Care duties as assigned.

QUALIFICATIONS.

Education. Master of Social Work degree preferred

Experience. Two (2) years' experience in a health care social work setting with knowledge and experience in hospice work preferred.

Licenses/Certifications. Licensed social worker in New Mexico preferred. Must have valid New Mexico driver's license. Current CPR certification.

Other (e.g., post-offer medical exam, polygraph, background check, driver's license record, etc.). Must pass a criminal background screening and driver's license record check. Must have reliable transportation as defined by a vehicle that is in good working condition and is readily accessible to the individual during work hours. Must have a clean driving record defined as no more than 3 moving violations in the last 3 years and no DUIS in the last 7 years. Must have valid auto insurance in place at all times and must provide proof of coverage. Understanding and support of the hospice philosophy of care.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED BY THE POSITION:

Ability to follow verbal instructions in English and to read/comprehend written instructions in English

Knowledge and understanding of spoken and written Spanish o Verbal and interpersonal skills to support customer relations and communicate with other members of the hospice team and our customers

Must exhibit highest degree of professionalism in appearance, manner, and interactions

WORKING CONDITIONS:

Operates in a professional office environment. Uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Frequently required to sit, stand, and walk. Frequent contact with employees and families in stressful situations. Ability to cope with stress created from deadlines, patient/family conflicts, regulatory issues, and interactions with customers and community at large. Occasional exposure to infectious diseases: ability to travel locally: some exposure to unpleasant weather and various types of family pets.
