Palliative Care Physician/Site Leader
Clinical Scholar Path, Assistant/Associate Professor

The Division of Hospice and Palliative (HPM) Medicine, within the Department of Family Medicine and the University of Vermont Health Network (UVMHN), is recruiting a 1.0 FTE board-certified/board eligible physician to join our growing palliative care program at the University of Vermont Health Network-Porter Medical Center (UVMHN-PMC) and the University of Vermont Health Network Helen Porter Rehab and Nursing (UVMHN-HP) and to also serve as Site Leader for the Porter Medical Center Program.

Porter Medical Center is a 25-bed critical access hospital and Helen Porter is a 98-bed skilled nursing facility on the same campus with a combination post-acute and long-term care beds. The hospital and SNF serve the residents of Addison and surrounding counties in Vermont as well as many residents of the Adirondack region of New York. Palliative Care consultation services offer an extra layer of support to patients and families navigating the multiple challenges of living with serious illness. We assist with goals of care, treatment decisions, symptom management, and psychosocial support.

Palliative Care services at Porter includes unique and dynamic volunteer programming. Palliative Support Service volunteers complement the clinical services provided by medical professionals by offering practical and emotional support to patients and families. These services include companionship, respite for family caregivers, around-the-clock vigil sitting during the last 24-48 hours of life, grief support during and after the dying process with bereavement companions and grief support groups, and community programming and education.

Our program was founded in 2017 as Porter joined the UVMHN. Over the years, the program has grown from a pilot project of 0.4 FTE MD time to its current composition of 1.5 FTE physician time and a 0.8 FTE palliative care certified LICSW position. In 2022, we became a site rotation for the UVMHN Hospice and Palliative Care fellowship where fellows spend time working with our team over two 4-week rotations.

The selected candidate’s clinical effort would primarily be caring for inpatient palliative care consults along with consults at Helen Porter and occasionally the outpatient or home setting.

In addition to clinical duties, the candidate will serve as Site Leader and build upon the significant growth in Porter’s Palliative Care Program, solidify the current commitment to inpatient and SNF work and help develop a plan for future growth of the program into the outpatient setting.
Additionally, they will also serve as site leader for the fellowship, determining clinical curriculum and participating in fellowship didactics/workshops and other administrative tasks. As a clinician-educator they will be encouraged to demonstrate a record of scholarly activity, as documented through invited lectures, quality improvement, curriculum development, or publications. The Division will provide support and mentorship to facilitate the candidate in meeting these goals.

The clinical appointment will be at the Assistant/Associate Professor level in the Clinical Scholar at the LCOM, corresponding to prior experience. The qualified candidate will possess experience in hospice & palliative care, excellent interpersonal communication skills, outstanding collaboration skills with interprofessional colleagues and other disciplines, and passion to improve the mission for palliative care.
