Description
Primary Job Responsibilities:
Perform bedside palliative care utilizing strong assessment and evaluation of the patient and their families
Collaborate with the patient's physician, other health care team members, the patient and their family in developing realistic care expectations based on goals of care
Facilitate patient and family decision making regarding treatment decisions
Prescribe pharmacological and non-pharmacological interventions
Monitor patient's response to treatment modalities and revise plan as needed
Assist in all facets of care coordination for palliative care referrals
Communicate effectively with patients and families for understanding of diagnosis, prognosis and treatment plans
Serve as a resource to healthcare team members regarding palliative care and end-of-life care management
Develop and maintain effective working relationship with providers, staff, leaders and community organizations to promote positive image; heighten awareness and promote collaboration
Conduct face-to-face encounters and complete documentation as required in a timely manner
Coordinate data and prepare outcome reports and statistics as required
Requirements:
Graduate of an accredited school of nursing and nurse practitioner program
Current license in good standing to practice in the Commonwealth of Pennsylvania
Minimum of two years RN or NP experience
ACHPN certification required or be able to obtain certification within 6 months of hire
Maintains clinical and professional competence
Possess a controlled substance license in the Commonwealth of Pennsylvania
Possesses the ability to communicate effectively with all members of the care team
Possesses good organizational skills
Current Pennsylvania driver's license and automobile insurance required
