Description

We are seeking a fellowship trained, BE/BC physician to join the Palliative Medicine program to provide inpatient and outpatient palliative medicine to patients with serious, chronic illness at our practice based at Temple University Hospital in Philadelphia.

KeystoneCare is a nonprofit, community-based organization providing full continuity of care by offering home health, hospice, palliative, private duty, waiver, therapy and telemonitoring services to patients and families of the Greater Philadelphia Region.

Full-time position with shared, remote call

Teaching opportunities with residents and fellow as well as medical students, nursing and CRNP students

Willing to provide grand rounds and didactics to improve palliative educations and awareness within the health system and community

Actively identify opportunities to improve the patient and family care experience

Improve the effectiveness of resources as used in management of symptoms, complex and chronic medical and social issues

Connect patients and their families to community hospice and palliative resources

Consulting with other interdisciplinary team members.

Serving as a liaison between the Palliative Care program and members of the medical community.

Assuming overall patient care responsibility.

Act as a consultant to staff for pain and symptom management interventions.
