POSITION SUMMARY:
The Advanced Practice Nurse (APN)-Palliative Care:
• Works in collaboration with the Palliative Care Medical Director, patients, their families, and others within the interdisciplinary team, as needed, in an active practice to deliver exceptional acute care and chronic medical management for patients with progressive illnesses.
• Serves as a professional and qualified provider, interdisciplinary consultant, and patient advocate with the responsibility to practice in accordance with the laws and regulations governing the practice of an APN in the state of Tennessee.
• Participates in daily team huddles/rounds, performs medical services in collaboration with palliative care physicians to diagnose and manage health care problems, and provides and documents medically necessary services in accordance with mutually acceptable physician/APN protocols.
• Promotes the involvement of the patients and their families in the plan of care with emphasis placed on pain and symptom management, as well as emotional, psychosocial, and spiritual support.
• Participates in clinical training, as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Participates with palliative care setting’s interdisciplinary team, as appropriate (i.e. morning rounds, care coordination, education and training, utilization committee, discharge committee, etc.).
• Develops the appropriate treatment plan that maximizes the health potential of the patient including, but not limited to:
o Ordering and interpretation of appropriate diagnostic tests within scope of practice;
o Identifying appropriate pharmacologic agents;
o Identifying appropriate non-pharmacologic interventions;
o Developing an education plan as appropriate;
o Interpretation of laboratory and radiology tests within scope of practice;
o Ordering of durable medical equipment.
• In collaboration with palliative care provider(s), refers patients to medical specialists or refers patients to other healthcare services as medically indicated.
• Consults with collaborative physicians, consulting practitioner(s), primary care physicians, and other specialist physicians as appropriate.
• Assists in all facets of care coordination for referrals.
• Provides disease management and education to patients and their families.
• Provides clinical guidance to facility staff relative to patient care issues, assessments, and interventions.
• Conducts initial and medical follow-up assessments, as well as performs reasonable on-call services as applicable.
• Works collaboratively with patients, their family and caregivers, physicians, interdisciplinary team, and other staff to facilitate effective transitions from one care setting to another.
• Comply with all applicable laws and regulations governing the practice of an APN in the state of Tennessee.
• Assists in problem-solving strategies with the patient, their families and caregivers, PCP, and setting’s interdisciplinary team to facilitate safe care of the patient.

JOB REQUIREMENTS:

• EDUCATION: Graduate of an accredited Nurse Practitioner program; Master's preferred. Additional training may be required for specialty areas. Qualified by education, training or experience to work with the adult and/or geriatric patient population as specialty assignment dictates.
• EXPERIENCE: Twenty-four (24) months clinical experience as a Registered Nurse. One (1) for more years experience as a Nurse Practitioner preferred. Palliative Care Experience preferred. Demonstrated ability to independently work and make clinical assessments. Additional experience may be required in assigned specialty area.
• LICENSE OR CERTIFICATION: Nurse Practitioner certification or registry eligible required. Current licensure as RN in State of Tennessee. Additional certification in specialty areas may be required (i.e., Family Nurse Practitioner, OB Nurse Practitioner, Occupational Nurse Practitioner). Eligible for Certificate of Fitness (Prescriptive privileges) in State of Tennessee required. BLS required.
• PHYSICAL DEMANDS: Constant walking, standing; lifting and positioning patients/equipment; exposure to infectious agents. Visual acuity is needed to access color changes. Heavy Work - Exerting up to 150 pounds of force occasionally and/or up to 75 pounds of force frequently and/or up to 30 pounds of force constantly to move objects.
