The Palliative Care Social Worker provides skilled assessment and intervention for patients of all ages with life-limiting illnesses or conditions and their families. Attends to family dynamics, assess and support coping mechanisms, and mediate conflicts. Works within the health care delivery network to foster best use of resources for effective service delivery and patient and family satisfaction. Ensures compliance with practice standards and codes. Advocates for interventions that alleviate burden and facilitate coping for Palliative Care patients and families. Participate in the Palliative Care Program’s interdisciplinary initiatives, such as the inpatient consult service, inpatient Palliative Care unit, outpatient clinics, bereavement services, learner education, and community projects.
