This position is a new line of service through transitions for Opioid Use Disorder with the UK HEALing communities.
Join our team to be a part of National Institute of Health’s historic HEALing Communities Study, awarded to the University of Kentucky, to reduce opioid overdose deaths by 40% in the next 3-years.


QUALIFICATIONS

Is a graduate of an accredited school of professional (registered) nurses, BSN preferred, experience with human subjects research preferred.
Is currently licensed as a Registered Nurse through the State Board of Nursing.
At least two year’s general nursing experience with addiction, home health, medical surgical, psychiatric, mental health care experience. Transition/Navigation nursing experience is preferred.
Understands evidenced-based transitional/navigation services to prevent 30-day avoidable hospital re-admissions, emergency room usage and improve patient outcomes.
Complies with accepted professional standards and principles.
Understands all Bluegrass Care Navigators’ lines of service, hospice, transitions, palliative care, Medicare Care Choices Model, adult day health care, private duty and personal services.
Must have excellent observation skills, good nursing judgement and communication skills. Ability to provide good written documentation in a timely manner.
Demonstrated ability to supervise and manage a team of navigation staff in the community and home.
Is self-directed with the ability to work with little supervision.
Must have excellent collaboration skills in working with UK HEALing Communities Study leadership and team.
Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles. Knowledge of disease processes, addiction medicine, symptom management, coaching process and community and health resources.
Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare and research team.
Current Kentucky driver’s license and unlimited access to a reliable automobile.
Ability to travel as needed to all study sites and partner organizations as needed.
Ability to exercise judgement and discretion in confidential matters. Trained in compliance with Health Insurance Portability and Accountability Act compliance. Will be required to train or have current completion certificate for Good Clinical Practice and Human Subjects Protections.
Must have excellent written and oral communication skills. Should be comfortable in public speaking with varied audiences.
Possess the ability to manage and interpret data.
Ability to interact with hospital/partner top leadership in regular meetings.
Experienced with coordinating clinical staff and providing effective education, feedback and direction.
Experienced with managing office support staff.
Must have strong organizational and planning skills.
Able to perform the essential functions of the position as listed in “physical requirements” section with or without a reasonable accommodation. Has presented a pre-employment health clearance including a negative TB skin test and/or CXR and other tests as required by organization policy.
Possesses and maintains active Kentucky RN license, Temporary Work Permit (TWP), Provisional License or active compact state RN license that is in good standing and American Heart Association (AHA) Basic Life Support certification for the Healthcare Provider valid for at least 6 months after employment date.
Must be flexible and can manage multiple tasks and programs simultaneously.

SUMMARY OF JOB RESPONSIBILITIES

An employee in this position assists in program development, management and evaluation. Is responsible for supervising transition/navigator staff, clinical and non-clinical. Responsible for coordinating all transition/navigation services in the community and home. The position manages the clinical navigation protocols for the Study, and is responsible for overseeing data collection, management and evaluation.


RESPONSIBILITIES AND DUTIES

Jointly supervised by the VP of Administration of BCN, UK HEALing Communities Study (HCS) designated team members utilizing a matrix model of management.
Responsible for assisting in the achievement of the UK HCS goal of using evidence based practices to reduce overdose deaths by coordinating and deployment of these practices according to instructions from the UK HCS team.
Assists in developing and coordinating relationships with partners, which includes, but is not limited to, hospitals, physicians, Opioid Treatment Programs, Syringe Exchange Programs, jails, etc. in order to increase education and distribution of naloxone; increase access to and retention in treatment programs utilizing medications for Opioid Use Disorder; encourage safe prescribing practices and encourage healthy conversations surrounding the disease to reduce the stigma of the disease and the medications used to treat the disease.
Responsible for developing and updating the program’s processes and procedures, under the direction of UK HCS team.
Assists in developing policies and protocols for the program.
Assumes clinical and care navigation duties as needed.
Coordinates referrals with community resources and continually looks for additional resources for navigators.
Supervises the UK HCS navigators, including annual evaluations that are inclusive of feedback and participation from UK HCS team members.
Audits charts for accuracy, compliance and provides feedback to staff.
Participates in the quality improvement program.
Works closely with the IT department on the electronic medical record development and ongoing changes needed for the program.
Routinely conducts conference calls, all-team emails with all navigators to review processes, provide new education materials and updates on the program.
Participates with Chief Financial Officer and VP of Administration in ongoing budget monitoring.
Identifies navigators that require additional training and education.
Attends HCS Study partner conference calls, in-person meetings and presentations.
Monitors accuracy of routine required submissions to partners.
Openly displays cooperation in adhering to administrative policies and supports the mission of Bluegrass Care Navigators and the University of Kentucky.
Assures that work and benefit hours are appropriately documented.
Reports to work as scheduled. Commits to maintaining adequate staffing through planned use of accrued leave, is punctual and maintains reasonable flexibility in work schedules.
Demonstrates a time-conscious attitude and consistently strives to use time effectively by completing assignments in an efficient, well organized, and self-directed manner.
Participates in interviewing and hiring navigator staff along with HCS designated team members.
Assists with regulatory compliance issues.
Conducts and represents oneself in a professional manner at all times.
Attends mandatory in-services.
Participates in organizational management meetings.
Assures that worked and benefit hours are appropriately documented.
Reports to work as scheduled. Does not abuse PTO, is not tardy, and maintains reasonable flexibility in work schedule.
Maintains dress code and evidences good grooming.
Other duties as assigned.
