Is a graduate of an accredited license practical nurse or vocational nursing program
Has two years nursing experience. Community health/hospice, medical/surgical or long-term care experience is preferred.
Is currently licensed as an LPN in the state.
Complies with accepted professional standards and practice.
Possesses and maintains good physical stamina and mental health. Has presented a pre-employment physician’s health clearance including a negative TB skin test and/or CXR and other tests as required by organization policy.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Excludes In-patient Unit Staff.
Understands philosophy of hospice concept. Also understands needs of the terminally ill.
SUMMARY OF JOB RESPONSIBILITIES
The Community Based Palliative and Primary Care LPN is responsible for the coordination of clinical components of the Community Based Palliative Care Program at Bluegrass Palliative and Primary Care. Acts as Coordinator for the palliative and primary patients and coordinates clinical components of the program throughout the palliative care continuum.
RESPONSIBILITIES AND DUTIES
Coordinates and case manages primary and palliative care patients seen in the Community Based Palliative and Primary Care Program of Bluegrass Palliative Care.
Provides clinical oversight of the delivery of care on a daily basis to assure quality services.
Collaborates with palliative physicians, nurse practitioners and other team members; acts as a liaison to the patient’s primary medical providers.
Works with Clinical Manager, Executive Director of Palliative Care and Chief Medical Officer to develop new policies for clinical components of the clinic, nursing home program, research, and other programs of palliative care
Ensures quality and safe delivery of services to patients/families/caregivers.
Provides patient/family/caregiver education and information on CBPPrC service lines and information pertinent to diagnosis and self-plan of care.
Educates the medical community on issues related to palliative care, especially at times of referral.
Coordinate the development and review of appropriate clinical protocols and processes that support appropriate referral and consistent care.
Assists the Clinical Manager and Executive Director of Palliative Care in standardizing clinical and clinical administrative practices throughout the palliative care program.
Works closely with Executive Director of Palliative Care, Chief Medical Officer, and other managers of Bluegrass Care Navigators to ensure that clinical protocols, utilization review, case management, quality assurance, and compliance programs are implemented according to corporate policies.
Assists with any home-based palliative and/or primary care clinical case manager needs.
Assists with palliative care education activities as requested.
Works closely with palliative and primary care leadership to enhance the palliative care program’s outreach to community physicians, the public, healthcare facilities and other applicable organizations in the community.
Participates in professional organizations in the areas of palliative and end-of-life care.
Openly displays cooperation in adhering to administrative policies and supports the philosophy, goals, and values of Bluegrass Care Navigators.
Demonstrates a time-conscious attitude and consistently strives to use time effectively by completing assignments in an efficient, well organized, and self-directed manner.
Shares information, knowledge, concerns, and expertise through staff meetings, and assisting in the orientation and education of new employees.
Establishes and maintains effective communication patterns using tact, sensitivity, sound judgment, and a professional attitude when relating to patients, patients’ families, co-workers, supervisors, health professionals, and physicians.
