The Hospice Physician, under the direct supervision of the Medical Director, is responsible for creating and maintaining the medical component of the Agency’s patient care program. This role serves as the Hospice Physician representative of the Interdisciplinary Team (IDT) and/or the attending physician for hospice and palliative care patients. The Hospice Physician is responsible for certifying (in conjunction with the attending physician, if applicable) that patients are terminally ill and re-certifying eligibility for hospice or palliative care for subsequent election periods.

Job Requirements
• Licensed Physician Controlled Substance with current DEA license number
• Hospice and Palliative Medicine Board Certification
• Current, or eligible, Medicare/ Medicaid provider number
• Current, or eligible, credentials at local hospital(s)
• Experience in hospice and palliative care
• Knowledge of health care regulations, policies and procedures
• Ability to demonstrate effective written and verbal communication skills and the ability to relate to various levels within the agency, to present to varied audiences and to facilitate group meetings
• Ability to demonstrate exceptional theoretical and practical knowledge to solve complex problems
• Ability to think creatively and develop recommendations to improve the effectiveness and efficiency of processes and systems
• Ability to function effectively in a fast-paced and changing environment with multiple priorities and objectives; to operate with a sense of urgency while maintaining flexibility and effectively managing priorities meeting deadlines
• Ability to maintain organizational confidentiality which includes, but in not limited to, information about patients/families, volunteers, employees, donors and agency processes
• Eligibility to bill insurance for palliative care and symptom management visits
