Description

Provides palliative care and consultations in an acute inpatient care setting. Functions as a member of the interdisciplinary palliative team to provide palliative services to patients with serious illness and their families. Team is made up of Physician, NP, Social Worker, and Pastoral Care. Assesses physical, psychological, emotional, spiritual and social needs of the patient and family. Makes recommendations for medications and other therapeutic forms of treatment in an effort to mitigate symptoms related to serious illness. Documents visits and discussions using an EMR system. Communicates findings and recommendations to the care team, including consulting and attending physicians. Participates in the discharge planning process to assure safe transitions across the care continuum. Keeps current on issues involving goals of care conversations, Advance Health Care Directives. Participates in staff and community education related to palliative care issues and end of life care. Relies on extensive experience and judgment to plan and accomplish goals. This is an exempt position. Must be able to bend, squat, lift and turn patients, able to lift 30 to 60 lbs, following industry standards and guidelines for safety and correct ergonomic practices. Employees involved in patient care may be exposed to blood, body tissue and fluids regularly and are expected to follow all health care industry safety guidelines indicated in our procedures manual.

Certification/licensure as an NP required. If not certified in Palliative Care, must be willing to go through the training process and gain certification in the specialty. 1 year experience as NP in inpatient setting and experience as NP or RN in Hospice/Palliative care or ICU setting
