The state-of-the-art, 192-bed MaineGeneral Medical Center at the Alfond Center for Health offers comprehensive inpatient and outpatient services for the surrounding communities. We’re located just an hour north of Portland, Maine’s largest city, and three hours from Boston.
In this out-patient/community position you will be joining the Hospice & Palliative Care teams. You will provide consultative and follow-up care to patients and families of the Hospice and Palliative care programs. The split would be 75% Palliative Care in Augusta at the Medical Center and 25% Hospice depending on the ebb and flow of Hospice patients.
You will practice in collaboration with the Hospice & Palliative Care Medical Director. We anticipate 40-60% office/home facility based care, providing recertification visits and consults to our patients and their families. There will be opportunity to provide inpatient consults/follow up support for palliative care and management for patients in the comfort unit in a 5-patient facility. Here, patients receive skilled care, long term care and hospice GIP level of service, all with comfort focused treatment goals.
Additionally you will be a resource to staff and families regarding palliative and end of life care partnering with the Medical Director. The ideal candidate will have a certification in Hospice and Palliative care, and a minimum of 3 years of experience. Requirements include: Maine NP license, 3 years of RN experience in a clinical setting, DEA, and valid driver’s license. Call is required every 4th weekend.
