Job Summary:

The Hospice Physician provides care as a consultant expert in palliative medicine and in some cases, as the primary physician at the request of other physicians or the patient. The Hospice Physician provides consultation and care as a member of the hospice interdisciplinary team in a variety of settings including patient's homes, hospitals, nursing facilities, the hospice residence, and hospice inpatient unit.

Job Duties

Provides all aspects of clinical care for hospice and palliative care patients and families.
Provides guidance and support as patients and families make their transition from curative to palliative care.
Provides competent and complete assessments and diagnoses.
Completes documentation and billing in the EHR in a timely manner.
Communicate information about diagnosis, prognosis, and treatment options to patients and families.
Provides guidance to patients and families during the process of making treatment decisions, so the patient’s wishes are honored.
Respects the patient’s beliefs, values and goals for care.
Provides skilled, effective interventions that meet the patient’s needs.
Collaborates with the patient’s attending physician and the interdisciplinary care team to achieve outcomes that met the patient’s needs.
Offers a caring presence to the patient and family and provide ongoing emotional support.
Supports the patient’s search for a renewed sense of meaning, purpose and hope.
Serves as an advocate to help patients and families receive needed services.
Participates in teaching and research activities to improve the standard of patient care.
Provides care to patients and families in a variety of settings including home, hospital, nursing facilities and the hospice residence and hospice inpatient unit.
Provides on-call support on an alternating basis, to the interdisciplinary team, patients, families and patient’s primary physicians in order to provide 24 hour,7 day per week medical care for our patients.
Adjusts therapies to meet the capabilities of family members and care givers and teach patient care techniques.
Provides support and works collaboratively with the interdisciplinary care team to meet the patient and family needs.
Participates in interdisciplinary team meetings and listens respectfully to help develop mutual trust and respect.
Asks for help from team members.
Serves as a resource to improve the team’s medical skills.
Pays attention and learns from other team members.
Helps the team stay focused on the patient’s and family’s problems.
Provides continuing education to health care professionals.
Provides organizational support.
Demonstrates careful stewardship of the organization’s resources to improve patient access to needed care.
Participates in performance improvement activities.
Assists with administrative tasks as directed by medical director.
Assists management in focusing resources where they are most needed to alleviate the patient’s and family’s suffering.
Communicates with community physicians about end-of-life care and hospice services in order to improve access to quality end-of-life care in Collier County.
Education And/Or Experience
Medical degree (M.D. or D.O.) from an accredited institution. Residency in internal medicine, geriatrics or oncology preferred.
Board Certified in Hospice and Palliative Care Medicine.
