Our Palliative Care Clinic in Prescott Valley is seeking a Master Social Worker to provide psychosocial support to palliative care patients as a member of the palliative care services consultation team. We will rely on you to apply your knowledge of psychosocial care in support of YRMC PhysicianCare and hospital quality improvement initiatives.

Requirements
• Master's degree in Social Work (MSW) from an institution accredited by the Council on Social Work Education
• Two years of clinical social work experience in a hospital, long-term care, outpatient or hospice setting
• Proficiency in Microsoft Office (Outlook, Word, Excel)
• Experience in palliative care or geriatric social work a plus
• Electronic Medical Record (EMR) experience preferred; Cerner highly desired
