Description

Elliot Health System (EHS) is seeking a Palliative and Hospice Nurse Practitioner to join our seasoned interdisciplinary team that has been steadily growing over the last decade. Our goal is to increase our outreach to the community and enhance patient care by also adding a clinical coordinator (RN) to the practice.
Our focus is driven by continuity of care with emphasis on pain and symptom management with exceptional empathy. As part of our commitment to ensuring continuity of care, our team sees patients at Elliot Hospital, multiple local Skilled Nursing Facilities and Assisted Living Facilities and in our new outpatient clinic.
Hours are Monday through Friday, no holidays. Qualified applicants will be energized, engaging, and forward thinking with a strong commitment to quality patient care. Work in a team environment in collaboration with our 3 Physicians, 2 Nurse Practitioners and Licensed Independent Social Worker.
