Hebrew SeniorLife is a non-profit, non-sectarian organization that provides housing and health care for seniors, research into aging, and education for geriatric care providers.

We are seeking a Director of Palliative Care. The Director will provide clinical administrative leadership for the Palliative Care Program at Hebrew SeniorLife, ensuring excellent patient care services, coordinating educational and outreach programs, and leading quality improvement initiatives. S/he will also provide operational leadership for all palliative care services, hospital based and in the community; provides clinical leadership and expertise within the palliative care clinical team; and proactively identifies opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use.

Responsibilities include:
• Oversight of the work of the palliative care clinical team members in three sites: NewBridge, Roslindale and Home and Community Based Services.
• Provide direct care in palliative care consultation as part of the interdisciplinary team.
• Participation and clinical leadership in the development and review of clinical protocols.
• Leads team meetings for the operational and administrative functions of palliative care program.
• Coordinates annual strategic planning for palliative care program.
• Development and oversight of metrics to demonstrate program impact.
• Oversight of program budget, review of performance measures, and use of staff resources proportional to activity and patient care.
• Regular review of financial and operational impact of palliative care program at three sites, and production of summaries for leadership review.
• Leadership in the identification of education needs for HSL staff and development of effective educational strategies.
• Outreach to community agencies and health care settings as appropriate.
• Assures that clinical protocols, case management and quality assurance are implemented according to HSL policies.
• Spokesperson for the palliative care program internally and externally.
• Works with HSL leadership to enhance the palliative care program’s community outreach to physicians, the public, healthcare facilities, and other applicable organizations in the community.
• Works with the Director of Development to secure philanthropic support.
• Co-management of Palliative Medicine Fellowship along with Medical Director of Palliative Care.
• This successful candidate should have a minimum of five (5) years of clinical and three (3) years of administrative experiences. Certified in Hospice/Palliative Care. At least one (1) year experience working as part of a hospice/palliative care interdisciplinary team. Experience managing and supervising multidisciplinary staff teams, program budgets and program development in hospice and palliative care. Management and leadership experience within complex healthcare, community based care or long-term care organizations preferred.

We are also seeking a Part/Full Time weekday Palliative Care Nurse Practitioner to join our robust interdisciplinary consultative palliative care team at our flagship campus in Roslindale, MA. No call or weekend coverage. The Nurse Practitioner will provide palliative care consultation in our long term chronic care and post-acute services. Responsibilities will include: conducting medical and psychosocial evaluation of patients and families referred for consult; facilitating family meetings when complex goals of care are being discussed; providing expert pain and symptom management; and serving as a clinical and educational role model and mentor to primary staff.

Candidate must possess strong clinical experiences caring for elders at the end of life, and patients with complex, chronic or life-threatening conditions. Outstanding interpersonal skills. Experience in palliative care and/or geriatric and/or palliative care certification preferred. Must be a graduate of a master's level state and NLN approved NP program. Possess appropriate state licensure to practice in the expanded role. Current license to practice professional nursing in the Commonwealth of Massachusetts. Maintain or be eligible for Mass DPH Controlled Substance registration and DEA registration.
