Apoyo con Cariño (Support through Caring) is a culturally tailored patient navigation program designed to improve access to and engagement with palliative care among Hispanic/Latino patients living with serious illness. Developed through close collaboration with community members, patients, families, and clinicians, the intervention addresses key barriers such as language differences, limited awareness of palliative care and hospice, and a lack of culturally responsive support. Trained lay navigators provide education, build trusting relationships, and support patients and families in managing symptoms, making informed care decisions, and accessing services that align with their needs and values.
