Overview

Clinicians from diverse disciplines, specialties, and settings care for people with serious illness every day—but many feel unprepared to provide their patients with pain and symptom management, communication, and more, which all people with serious illness deserve.

In this webinar, Drs. Wendy Anderson, Carin van Zyl, and Julie Freedman will share their experiences implementing palliative care into a range of settings and specialties in health care systems in the state of California. They will discuss collaborations between palliative care and partner service lines (e.g., primary care, radiation oncology) to identify gaps in the care of people with serious illness; corresponding interventions to close these gaps including pain and symptom management, advance care planning, and goals of care conversations; and key project outcomes.

Participants will learn how to plan and implement palliative care partnerships to improve the care of people with serious illness, and how to access free tools available from the California Health Care Foundation (CHCF) to support these efforts. By the end of the session, participants will be able to describe a range of opportunities, a needs assessment process, and more.
