Exploring Best Practices in Serious Illness Care for Medicare Advantage and ACOs
The Center to Advance Palliative Care (CAPC), working with a group of 43 organizations from across the country, has released summary guidance for Medicare Advantage plans and Accountable Care Organizations (ACOs) seeking high value care for people living with serious illness. Building from the advice provided in the 2017 Serious Illness Strategies, these organizations worked through four key strategies: early identification of the population in need; strengthening care management capabilities for this population; supporting access to home-based care; and incentivizing improvements in network capabilities.
