Brook Calton, MD, MHS

Dr. Brook Calton is an Associate Clinical Professor in the UCSF Division of Palliative Medicine. She is an outpatient palliative care specialist, program developer, and educator. Dr. Calton is the founding medical director of UCSF’s first home-based palliative care program - which utilizes a fee-for-service reimbursement model to care for seriously ill San Franciscans at home. She is also the founding medical director of By the Bay Health, a home-based palliative care program shared with UCSF’s affiliated hospice,Hospice by the Bay. By the Bay Health is funded via payer-provider partnerships and is currently seeking Joint Commission certification. Dr. Calton aims to use the programs she leads to: provide the highest quality care to patients and their families, serve as a model for national palliative care programming, and function as a platform for training the next wave of practitioners. She spends her free time with her husband and two small children exploring the beauty of San Francisco and searching for the best pizza in the Bay Area.
