Sanitas Palliative Care Program is in search of a Social Worker (MSW) for our Miami, Florida Region. The candidate may be licensed or have a supervisor. Our Program covers 11 counties in South, Central and West Florida. Our multidisciplinary team consists of a Director of Palliative Care Operations (nurse), Medical Director, Business Development, APRNs, MAs, MSWs, Spiritual Care, Outreach, Case Manager and Behavioral Health. This is a home program and the MSW drives to homes for appts with patients and families. We also have the option to see patients in one of our many clinics or televisit if necessary. The MSW assists with patients and families dealing with chronic and serious illness including difficult conversations/situations. The role includes emotional support for patients/families/caregivers, application assistance for benefits (Social Security/Disability, Medicare/Medicaid), rental assistance, section 8, shelters, food assistance (SNAP, food pantries, farm share, and community resources), transportation assistance (bus, passes for rideshares), utilities, grants (community grants to assist with costs), and community donations. The MSW is an essential member of our team and works closely with the providers in their region. The MSW works from their home when not conducting in-person visits, online and in-person meetings are required. While remote, our team is close and we communicate frequently throughout the day with rapid support.
