Home-Based Primary Care Physician Opportunities: Lousiville, KY | Lexington, KY
Bluegrass Care Navigators is seeking a Home-Based Primary Care Physician to provide comprehensive, relationship-based care to patients where they live.
We currently have a priority opening in Louisville, with an additional opportunity in Lexington. Physicians provide care to patients in private homes, assisted living communities, and personal care facilities.
Position Summary
The Home-Based Primary Care Physician provides direct medical care to patients with chronic and acute health needs while helping medically complex patients receive high-quality care in the setting they call home. The physician also serves as a clinical resource and provides support and oversight to Nurse Practitioners within the home-based primary care program.
Responsibilities
* Provide comprehensive primary care in patients' homes and residential settings
* Evaluate and manage acute and chronic medical conditions
* Order and interpret diagnostic testing and prescribe medications
* Coordinate referrals and collaborate with specialists and the interdisciplinary care team
* Provide clinical support and supervision to Nurse Practitioners
* Perform procedures within the physician's scope of practice and available resources
Schedule
Monday–Friday with occasional on-call responsibilities.
Qualifications
* M.D. or D.O. required
* Kentucky medical license, or eligibility for licensure
* DEA registration and controlled substance authority, as required
* Board certified or board eligible
* Geriatrics training or experience preferred, but not required
* Experience in primary care, geriatrics, home-based care, hospital medicine, or caring for medically complex patients preferred
* Valid driver's license and reliable transportation required
