Atlantic Health is Seeking an Ambulatory Palliative Care Physician

Atlantic Health is seeking a compassionate and collaborative full-time Ambulatory Palliative Care Physician to join our growing palliative care team across multiple outpatient sites in Northern New Jersey.

This physician will provide specialty palliative care embedded directly within oncology and primary care practices, delivering longitudinal, relationship-based care for adults living with serious illness. The role focuses on symptom management, goals-of-care alignment, advanced care planning, and comprehensive support for patients and families throughout the disease trajectory.

The Ambulatory Palliative Care Physician serves as an integrated member of oncology and primary care teams, offering real-time consultation, shared visits, and coordinated care planning to improve quality of life while patients continue disease-directed treatments. The ideal candidate will have strong symptom management skills and provide services primarily in the outpatient setting, coordinating closely with primary care providers and specialists.


Responsibilities

Provide outpatient palliative care consultations and longitudinal follow-up care
Manage complex symptoms associated with serious illness
Lead goals-of-care discussions and advance care planning conversations
Support patients and families through difficult medical decision-making
Collaborate closely with oncology, primary care, hospital-based teams, and specialists
Participate in interdisciplinary care planning to ensure seamless, compassionate care delivery
Promote patient-centered care that aligns treatment plans with patient values and goals

Required

Board Certified or Board Eligible in Hospice and Palliative Medicine
Board Certification in Internal Medicine or Family Medicine required
NJ licensed or eligible for licensure
Strong communication and interpersonal skills
Demonstrated expertise in advance care planning and serious illness conversations
Experience in oncology, primary care, or ambulatory serious illness care
Excellence in communication, shared decision-making, and interdisciplinary collaboration
Commitment to compassionate, high-quality patient care in a collaborative environment

Preferred

Prior experience in embedded or co-management palliative care models
Familiarity with NJ POLST statutes and end-of-life care regulations
Interest in program development, education, or ethics-based consultation
Comfort working in diverse urban, suburban, and multicultural communities
