Location: Wisconsin (regional coverage across Fond du Lac, Ripon, Waupun, Baraboo, Columbus, Madison, Janesville, and Monroe) | FTE: 1.0
Practice Setting: Inpatient, outpatient, and home-based community visits | Team: 2 Physicians, 11 APPs, and 15 support staff
Overview
SSM Health is seeking a Hospice & Palliative Medicine Regional Medical Director to join our compassionate team in Wisconsin. This leader will support inpatient, outpatient, and home-based community visits across the region while partnering with an interdisciplinary care team to deliver comprehensive, patient-centered care—and help shape the future of palliative care in Wisconsin.
Key responsibilities
• Provide regional medical director leadership across inpatient, outpatient, and community-based palliative care and hospice services in Wisconsin.
• Serve as the collaborating provider for Hospice & Palliative Care Advanced Practice Providers.
• Lead program initiatives and partner with administrative leaders, clinical staff, and service lines across the Wisconsin Region.
• Define, track, and improve quality and safety metrics for hospice and palliative medicine services.
• Help develop, expand, and standardize the Regional Hospice & Palliative Medicine Program, including outpatient and telemedicine care.
Qualifications
• MD or DO.
• Eligible for and able to obtain a Wisconsin medical license.
• Board certified in Hospice & Palliative Medicine.
• Minimum of 5+ years of post-fellowship experience.
• Demonstrated ability to lead collaboratively within an interdisciplinary team environment.
Schedule & role breakdown
• 0.2 FTE: Palliative Medical Director duties.
• 0.2 FTE: Hospice Medical Director duties.
• 0.6 FTE: Dedicated to palliative care outpatient and telemedicine care.
