Starting Wage: $38.35-$52.74 per hour, depending on experience

Position Summary:
Our Palliative Care LCSW (Licensed Clinical Social Worker) supports Asante palliative care through telemedicine by collaborating on patient care as a part of an interdisciplinary care team-based approach with physicians, other providers, nurses, and health care professionals with a goal of improving health care quality. Responsible for providing subspecialty palliative care counseling to inpatients at Asante hospitals via telemedicine, independently and in collaboration with medical providers. Fields and conducts patient calls as needed. Assist in shaping and directing palliative care in multiple settings by providing palliative care education to providers and staff throughout the system, and providing project management for organization-wide palliative care initiatives.

Qualifications:

Education
- Master's degree in Social Work, required
- PhD in Social Services, Healthcare Management, Psychology, or related field, preferred

Required Experience
- 10 years of LCSW-based social work, preferably in subspecialty palliative care
- Experience with telemedicine
- Experience with psychosocial assessment of physical symptoms and function (including use of ESAS, PPS, ECOG, Karnofsky)
- Knowledge of care planning in chronic disease, particularly CHF, COPD, dementia, and cancer, including the ability to discuss burdens/benefits of medications commonly used for disease management and symptom control.
- Expert level ability helping families cope with cognitive dysfunction (dementia, delirium, encephalopathy, coma), facing patients with life-limiting illness.
- Ability to educate patients and families on symptoms seen in last hours of life, and to coach staff in patient/family support in last hours.
- Can discuss and offer non-pharmacologic modalities for EOL symptom management.
- Expert level assessment (including use of MOCA, SLUMS, CLOX) and assessment of decision-making capacity , individual therapy, evidence-based counseling, and crisis intervention.
- Experience working in the medical social work or community, mental health field, and knowledge of Integrated Behavioral Health in the community healthcare system to include an understanding of the benefits/burdens/cost/delivery structure of interventions and living situations commonly offered patients with life-limiting illness (enteral and intravenous feeding, tracheostomy, intubation, long and short term ventilation, sensory and motor adaptive equipment and tools, chemotherapy, radiation therapy, SNF/ALF/AFC/ICF care).

Licensure/Certifications:
- LCSW: Licensed Clinical Social Worker by the State of Oregon, required upon start
- PHSW-C: Advanced Palliative & Hospice Social Work Certification, preferred
