Full-time - Remote - flexible schedule
Hours typically 8am – 4:30pm
Shared call 1:4 - 1:6
As a Hospice physician on our team you will:
Evaluate patients for qualifying terminal diagnoses and continued eligibility for hospice care.
Collaborate with on-site team to develop personalized treatment plans, manage the ongoing care of patients, and monitor progress
Provide evolving symptom management, medication stewardship, and controlled substance monitoring
Manage acute outpatient concerns and complications of our home hospice patients
Primarily remote, however there may be a need for in-person patient contact from time to time, in-person team meetings, as well as attendance at interdisciplinary group (IDG) meetings per Medicare requirements
Facilitate the transition for patients needing General Inpatient (GIP) or respite care to appropriate facilities
You will work collaboratively with experienced and compassionate staff, team of nurses, and specialists at each location
EPIC EMR
Be part of an integrated nationally recognized organization with physician-led medical group and embedded service line
Leverage access to DAX Copilot, an AI-powered clinical documentation tool integrated with Epic, streamlining clinical workflows by generating draft patient notes for efficient review, editing, and signature
Experience an environment that provides safe and equitable care for all patients
Training and/or Experience Required
Completion of an accredited Residency Program
Compassionate physician with a strong understanding of the hospice philosophy of care and expertise in hospice medical management
BE/BC
Primarily remote however must be able to come in for in-person team meetings and potentially face-to-face visits with patients
Medical Director Certification is preferred however not required; will be an expectation to take the exam within a few years after start of employment
Ability to obtain IL licensure
