Description
Whether you’re looking for a new opportunity or a fresh start, there has never been a better time to join the Optimizing Advanced Complex Illness Support (OACIS) Palliative and Hospice Medicine team — part of Lehigh Valley Health Network (LVHN) and Jefferson Health.
Practice Overview:
Lehigh Valley Hospice, now part of Jefferson Health, is an integral part of our health network and the community we serve. We provide comprehensive physical, emotional, social and spiritual care for patients and their families during the final stages of illness, throughout the dying process and into bereavement.
Our specialty-trained interdisciplinary hospice team includes physicians, CRNPs, nurses, social workers, spiritual counselors and dedicated administrative staff – all working together to deliver compassionate patient-centered care. As a hospice physician leader, you will foster a supportive, collaborative environment that empowers the team to be the eyes, ears, hands and heart of the physician at the bedside.
Education is at the core of our culture: We host two ACGME fellows per year who rotate with our team, contributing to a rich learning environment. As the region’s leader in hospice care, we are committed to providing patients and families with peace, comfort, courage and dignity – whether in their own home or in our inpatient hospice unit. We are also deeply committed to advancing health equity and delivering culturally sensitive care to every patient we serve.
In a medical director of hospice career with LVHN, you will:
• Provide Strategic Oversight of Hospice Operations: Lead initiatives to ensure exceptional quality, patient safety and regulatory compliance across all hospice programs.
• Serve as Physician Leader for Interdisciplinary Teams: Collaborate with care teams, attend biweekly interdisciplinary group meetings and perform home visits as needed to support patients and families.
• Round on Hospice Inpatients: Deliver compassionate care through both in-person and telehealth visits, ensuring timely and coordinated treatment plans.
• Participate in Shared Call Schedule: Join a fair rotation of on-call and holiday coverage, supported by a collaborative group of hospice physicians.
