Out Patient Palliative Care FT Monday - Friday
Provides palliative care consultations and follow-up visits in Post-Acute care settings; Home, ALF. In-person and Virtual visits. Coverage assistance in ambulatory Supportive Care Clinic located within system Cancer Center
Functions as a member of the interdisciplinary palliative team to provide palliative services to patients with serious illness and their families. The team is made up of physicians, NP, Pastoral Care, and available social work support.
Assesses physical, psychological, emotional, spiritual, and social needs of the patient and family. Manages and makes recommendations for medications and other therapeutic forms of treatment to mitigate symptoms related to serious illness. Documents visits and discussions using the EPIC EMR system. Communicates findings and recommendations to the care team, including consulting and attending physicians. Close collaboration with the inpt palliative care team, RSFH Home Health, and RSFH Hospice programs
Keeps current on issues involving goals of care conversations, and Advance Health Care Directives. Participates in staff and community education related to palliative care issues and end-of-life care. Relies on experience and judgment to plan and accomplish goals.
Certification/licensure required; Certification in Palliative Care preferred. 1 year experience as NP required and experience as NP or RN in Hospice/Palliative care desired.
