Advanced Illness Care Program: Utilizing a ‘Five Cornerstone’ Approach

Summary
Research demonstrates that care navigators can improve outcomes for individuals living with serious illness. To address persistent disparities in care, AC Care Alliance developed the Advanced Illness Care Program (AICP), a community-based model rooted in the trust and influence of the African American church. The program trains community-based care navigators to provide outreach, education, and holistic support that aligns with patients’ cultural, spiritual, and personal values.
Recognizing that serious illness affects every aspect of life, AICP uses a Five Cornerstone framework that addresses spiritual well-being, health care needs, advance care planning, social needs, and caregiver support. Through this comprehensive approach, the program helps advance more equitable, person-centered care.
