

Home and community-based palliative care programs have the special ability to meet more patients where they are. Though these programs help expand the reach of palliative care, they also face unique operational, financial, and clinical challenges.
In this webinar, Praba Koomson, DNP, will share how Sutter Health’s Advanced Illness Management (AIM) program, a nationally recognized model that provides interdisciplinary support to patients with serious illness, has tackled these complexities while delivering care across a large and diverse population. Dr. Koomson will demonstrate how AIM has turned the challenges of community-based palliative care into opportunities for innovation, equity, and improved patient outcomes.
By the end of the session, attendees will know how to identify key design and operational strategies to deliver effective community-based palliative care. They will also be able to analyze common challenges in implementing and sustaining home-based palliative care programs and apply lessons from the AIM model to enhance integration, workforce development, and care coordination in their palliative care program.
