Innovative Initiatives in Community-Based Palliative Care
Overview
Community-based palliative care is delivered in the home, the clinic, and in long-term care settings. Programmatic challenges include workforce shortages, team member well-being, lack of resources (financial, personnel, and clinical) and sustainability. However, palliative care programs are creatively providing quality, patient-focused services in the community based on their individual assets.
This session will highlight three programs that have created community-based initiatives that promote access to quality, interprofessional palliative care in the community that is both replicable and sustainable; Duke University (care in the clinic); Lehigh Valley (interprofessional education); and Medstar (effective post-discharge care).
