Housecall Providers, an organization in Portand, Oregon, provides care for the city's most vulnerable residents — the homeless. Through Medicare, Medicaid, or dual eligibility, patients are matched with a primary staff member, and "through that relationship, these economically and psychologically vulnerable patients learn to trust, and to realize, from the care they receive, that they matter as human beings."
