The American College of Physicians Lays Out Recommendations for Better Care Coordination
On April 29, 2022, the American College of Physicians (ACP) released a position paper with recommendations for more effective collaboration between primary care and specialty physicians with the goal of improving patient care. The paper, Beyond the Referral: Principles of Effective, Ongoing Primary and Specialty Care Collaboration, explains the importance of care coordination and outlines recommendations for improvement, with four guiding principles: 1) Patient and family partnering; 2) Defined clinical roles and responsibilities; 3) Timely, productive communication; 4) Effective data sharing.
