Today, CAPC launched Health Equity in Action, a new curated collection highlighting evidence-based interventions designed to advance equitable care for people living with serious illness. Drawing on successful health equity initiatives, the collection showcases practical examples that organizations can adapt to improve quality of life for populations experiencing disparities, connect with project champions, and drive meaningful change.

Health Equity in Action will be showcased during the open-to-all webinar, From Intervention to Impact: Scaling Equitable Care for Hispanic Patients with Serious Illness, on Tuesday, September 22 from 4-5 PM. The webinar will highlight replicable interventions that improve care experiences for Hispanic patients living with serious illness and their family caregivers, in honor of National Hispanic-Latino Heritage Month.
