Medical Society of the State of New York (MSSNY)
This activity has been planned and implemented in accordance with the Accreditation Requirements and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of MSSNY and Center to Advance Palliative Care. MSSNY is accredited by ACCME to provide continuing medical education for physicians.
The Medical Society of the State of New York designates this live activity for a maximum of 14.0 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
As an ACCME-accredited provider, MSSNY is required to identify and mitigate relevant financial relationships of all individuals in control of CME content.

Sachin Jain, MD, MBA, Speaker/Faculty for CAPC National Seminar 2025, has disclosed a financial relationship in stock (stock options) with the following Ineligible Company’s including: BioFourmis, Cherish Health, and Exact Sciences. Dr. Jain further disclosed that he owns stock (publicly traded) with GE Healthcare, Merck, and Vertex.
None of the other individuals in control of content have relevant financial relationships to disclose.
All relevant financial relationships have been mitigated.
Physician Assistants
The American Academy of Physician Assistants (AAPA) accepts credit from organizations accredited by the ACCME. The Medical Society of the State of New York designates this live activity with 14.0 AMA PRA Category 1 Credit™ as specified, which can be applied to the continuing education of Physician Assistants. Physician Assistants should claim only the credit commensurate with the extent of their participation in the activity.
Connecticut Nurses' Association
This activity has been submitted to the Connecticut Nurses' Association for approval to award contact hours. The Connecticut Nurses' Association is accredited as an approver of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation.
Social Workers
Center to Advance Palliative Care SW CPE #0257 is recognized by the New York State Education Department's State Board for Social Work as an approved provider of continuing education for licensed social workers effective 9/15/2025 - 9/17/2025. Social Work CEs for Seminar are under review.
How to Earn Contact Hours
To successfully earn credit for a session, participants must attend 90% of the live activity. Participants who wish to receive CE/CME credit must scan their badge upon entry into each session within 10 minutes of the start of the session to meet the 90% attendance threshold for the session. If participants do not scan their badge upon entry they will not receive credit for the session. Participants must also complete the post-session evaluation in order for credit to be awarded. Physicians must also complete the CAPC National Seminar 2025 Physician Claim Form in its entirety and submit the form electronically within two weeks of the Seminar to receive credit. A printable certificate of completion or a continuing education award document specific to the attendee’s discipline will be awarded.
