The Lilian and Benjamin Hertzberg Palliative Care Institute of the Mount Sinai Medical Center, School of Medicine, located in New York City, is seeking a palliative medicine clinician-educator. We are seeking the best qualified candidate for a palliative medicine Fellowship Director position.
The Mount Sinai School of Medicine is internationally recognized for innovative approaches to medical education and research. It is seamlessly connected to Mount Sinai Hospital, one of the county’s oldest and largest voluntary teaching hospitals. The School’s Hertzberg Palliative Care Institute is an international leader in the field of palliative medicine.
The palliative medicine physician will spend approximately 50% time working with the interdisciplinary team comprised of nursing, social work, massage and yoga therapists and a chaplain. The balance of time will be used for career development, education, program development and administration consistent with the expertise of the selectee. Our client is Mercy Medical Center-North Iowa and their wholly owned subsidiary hospice – Hospice of North Iowa. Mercy Medical Center-North Iowa is a financially stable, not-for-profit Medical Center nationally recognized as a Top 100 Cardiovascular Hospital by Thomson-Reuters Healthcare.
Seventy five percent (75%) of the time will be spent leading the well established Hospice Program. Hospice of North Iowa has a 6-Bed freestanding Hospice House. ADC for home patients is 96 and ADC for hospice house patients is 3.2.
Twenty five percent (25%) of the time will be involved with the Palliative Care program at Mercy-North Iowa. The Fellowship Program Director (Palliative Care physician) will report up to this CMO role. A focus will be continuing to grow the Palliative Care service which currently averages 700-800 consults a year.

Team Medical Director
Nashville TNResponsible for medical quality and continuity and delivery of services. Develops clinical and support staff. Ensures clinical standards and outcomes are met. Ensures compliance with federal and state regulations, JCAHO and NHPCO standards. Performs in a manner which aids the agency in meeting its Mission.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Reviews medical records for eligibility of hospice admission or continuation of services for the patient according to N.H.P.C.O. established guidelines and agency criteria. If patients are not believed to be hospice eligible, the charts are then reviewed by the CMO and COO before patients are discharged from hospice.
- Collaborates in developing interdisciplinary plan of care with attending and consulting physicians as well as IDT.
- Makes provisions for/attends team meetings.
- Makes patient visits when indicated for acute problems, new patients and per family/patient request. It is expected that home team medical directors make an average of 16-20 visits per week for hospice and palliative care patients. If assigned to an inpatient unit, physician helps with home and palliative care consults as time allows.
- Participates in 24 hours a day, 7 day a week on-call physician coverage. In addition, he or she is available to staff unable to reach the attending physician or designated physician on-call.
- Serves as a medical resource to hospice clinical staff via phone or beeper.
- Provides one weekend of call coverage per month.
- Serves as a liaison between staff and attending physician.
- Serves as a hospice representative to the medical community to inform physicians and the other health care professional of the availability and scope of agency’s services.
- Collaborates with the CMO, Clinical Coordinator, IDT, and COO to continually improve the quality and cost effectiveness of care and to enhance the work lives of the employees.
- Participates in education of nursing students, medical students and residents.
- Participates in community education programs, professional organizations and legislative activity as needed.
- Maintains and develops knowledge of evolving hospice and palliative care philosophy and practices.
- Promotes the advancement of hospice knowledge by participating on the appropriate committees of the State and National Organizations.
Manages Nurse Practitioners. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.

Hospice of Kitsap County is seeking candidates for a full time Medical Director position. The Medical Director is a core member of the Hospice team and is responsible, for overseeing patient care in the inpatient and outpatient settings. The Medical Director develops, maintains and monitors all policies and standards of the medical activities of the Hospice program to insure compliance with federal, state, and national standards of hospice care. Assumes medical care of hospice patients in emergency situations and when the attending/primary care physician is not available. Provides full-time medical care of inpatient hospice patients as needed. Is available to perform core Medical Director activities (confirmation of patient eligibility and appropriateness for hospice services upon admission and on an on-going basis, oversight and direction of additional IDT’s, authorization of patient services, and consultation in regard to ethical issues). Provides education to staff regarding clinical medicine in end-of-life care to staff.
Successful candidates will have:
WA State Medical license and completion of residency program. Experience in palliative care medicine, geriatric medicine, and/or care of the terminally ill preferred. Previous medical leadership experience highly desirable. Previous teaching experience highly desirable. Understanding and acceptance of hospice principles of care. The Palliative Care Center of the Bluegrass (PCCB) located in Lexington, has been recognized as a program of excellence by the Center to Advance Palliative Care in being named one of six national “Palliative Care Leadership Centers”. As a member of an interdisciplinary team with a chaplain, social worker, and physician, you will share the responsibility of new consults and on-going patient care in an inpatient setting. The advanced practice nurse plays a key role in ensuring excellent clinical outcomes for patients referred to the consult service.
This is a part-time (.75), benefit-eligible position, with potential for full-time hours as the program grows. The palliative care team generally consults with patients and families during morning and afternoon hours, however, the exact schedule is flexible and not yet determined. Our ideal candidate will have prior palliative care experience, or experience in areas that could include geriatrics, med/surg, oncology, critical care or family medicine. Additional education in palliative care and support for certification will be provided.
Patients will primarily be seen at Fairview Southdale Hospital. Full Time Palliative Care Social Worker
Roles and responsibilities:
- SW will be part of the new expansion of the outpatient palliative care services which aims to implement an interdisciplinary model of care including assessment by a MD or NP and a full assessment by the clinical Social Worker for patients seen in the clinic. It is expected that all patients seen in clinic will have full documented psychosocial assessment.
- SW will be responsible for starting and championing a bereavement program for families of patients who have died and were seen by the palliative care team.

This new position will have the major responsibilities of: Identifying and coordinating hospital-based palliative care and hospice referrals; ensuring that physicians, nursing staff, and case management staff understand palliative care and hospice care and the resources provided to the patient outside the hospital; monitoring and reporting outcomes of referrals to palliative care and hospice.
Requires either a BSN (MSN preferred) with license to practice in Virginia, or a Master’s prepared Social Worker.
The ideal candidate will have a minimum of 2 years of public/community health, medical /surgical, or hospice nursing/social work experience. Hospital based nursing/social work experience and certification in hospice and/or palliative care is preferred.
Must have advanced communication, counseling, and interpersonal skills with all levels of internal and external customers including medical staff, patients and families, clinical personnel, support and technical staff, outside agencies, and members of the community. Other needed skills include excellent written/verbal communication skills, critical thinking, creative problem solving, proficient organization and planning skills.

Hospice Medical Director
Hospice & Palliative Care of Cabarrus County, a leader in end-of-life care in Cabarrus County for 25 years, is seeking a full time Medical Director. HPCCC is located just five minutes from the new Bio-Tech Research Campus in Kannapolis, 15 minutes from Charlotte Motor Speedway and 30 minutes from downtown Charlotte. The Hospice Medical Director will oversee the medical component of hospice services, provide direct services to hospice and palliative care patients in a variety of settings including the Bob & Carolyn Tucker Hospice House, provide medical consultation to the Hospice Care Team and area physicians, promote hospice and palliative medicine throughout the community, including both formal and informal physician education. Additional responsibilities include medical oversight of inpatient palliative care services at CMC-NorthEast, performing physician consults and medical supervision of the Palliative Care Nurse Practitioner.
This position requires a current MD license in North Carolina, ability to travel off site to make patient visits, and the understanding of and ability to advocate for the Hospice philosophy of care. Health System Chief Palliative Care Officer
The sponsoring organization is the flagship hospital of the largest health system in northwest Florida. The service is well established (almost eight years old), and has helped redefine how late-life care is provided in the 458-bed flagship hospital of the regional health system. This service has generated significant interest among key stakeholders, who are eager to integrate these palliative care protocols into their clinical practices given greater physician support.
