Palliative Care Nurse Practitioner (APRN)
Location: Lexington, KY
Schedule: Monday-Friday 8am-5pm; 1 weekday after-hours on-call & approximately 9 weekend calls per year.

As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the role:

The APRN exemplifies and promotes advanced nursing practice and excellence in palliative care. The Palliative Care APRN provides comprehensive, quality care to individuals living with serious illness. The APRN will focus on providing relief from the symptoms and stresses of a serious illness with the goal of improving the quality of life for patients, caregivers, and families. The APRN will be responsible for providing visits for both new and established patients which may include visits to patient’s homes, facilities, and hospitals. The APRN works in close collaboration with both community and Bluegrass Care Navigators' (BCN) physicians, as well as our community partners through established palliative symptom management protocols. The APRN plays an integral role in formulating a plan of care for the patient/family/caregiver and assists with facilitating clear communication of this plan to BCN's physicians, the patient’s primary care provider, or specialist as appropriate. The APRN could help develop practice protocols for palliative and end-of-life care or educate other clinicians about the palliative care specialty all while maintaining one’s own professional knowledge and skills as an expert and role model in BCN advanced nursing practice. Additionally, the APRN will be responsible for performing other duties as assigned including but not limited to coverage of additional service lines and locations.

Requirements
Is a graduate of a Master’s Degree in a Nursing Program. APRN certification as Family or Adult Nurse Practitioner. In-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective practice.
Is currently licensed as an APRN through the State Board of Nursing.
At least three (3) years general nursing experience is required with current hospice, palliative care, home health, medical, surgical or critical care experience. Has experience with providing good pain management. I.V. skills preferred.
Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
Committed to the research process and application to practice.
Ability to organize resources appropriately.
Strong interpersonal skills, communication, collaboration and conflict resolution.
Knowledge of systems and the change process.
Ability to manage multiple priorities simultaneously and effectively handles the emotional stress of the workload.
Complies with accepted professional standards and principles.
Possesses and maintains current CPR certification.
Have satisfactory references from nursing school, previous (or current) employers, and/or professional peers.
Has excellent observation, good nursing/medical judgment and communi­cation skills. Ability to provide good written documentation in a timely manner.
Is self-directed with the ability to work with little supervision.
Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
Must be a licensed driver with automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
